    Responsibilities

    Assesses psycho-social needs of the ESRD patient and provides assistance to patients and their significant others to meet these needs. Works with the renal interdisciplinary team to formulate an individualized plan of care for each patient. Provides a psycho-social assessment in the medical record and updates social work notes regularly. Organizes and conducts patient and family support groups. Directs patients to appropriate community resources.  Flexible, cross train in chronic and acute units.

    Qualifications

        Master's Degree in Social Work.

        Current New York State license to practice social work.

        Knowledgeable in federal, state and local regulations.

        Knowledge of outside health-related and community resources for ESRD patients (preferred).

        Interpersonal skills necessary to form a productive relationship with multiple patients and significant others - to conduct effective support groups.

        One year experience as a Social Worker (preferred).

        Must have the ability to walk, move, stand for extended periods.

        Must be able to lift and carry a minimum of 0 to 50 pounds.

        Will be cross-trained to WMS and to the Acute Unit to meet the needs of the department.
