    Responsibilities

    This position: demonstrates competency with compression application, NPWT and assisting physician with wound care procedures; executes professional nursing care in accordance with Physician treatment plans in order to provide for the comfort and well-being of patients and their family or support system. In doing so, assesses the patient’s needs, plans and implements the treatment plan, observes and evaluates the patient’s progress and documents pertinent observations.Works with the patient’s physicians to assess the suitability of antibiotic therapy, surgery or hyperbaric oxygen therapy. Seeks other information as needed, including collaboration and consultation with physicians and other clinicians for evaluation of illnesses and managing acute and long-term medical needs of patients. Provides monitoring and continuity of care between physician visits, triaging patient telephone calls and providing consultation. Works collectively with the interdisciplinary health team to meet the holistic needs of the patient. Participates in Performance Improvement activities and committees to further improve quality of patient outcomes for the organization.

    Qualifications

    Employee Applicant must possess a current New York State License to practice as a Registered Professional Nurse. BLS certification required. 1 year of Outpatient Wound Care experience required. Wound Care certificate and HBO certification (e.g CWON, CWCN, and CWS) preferred or equivalent experience.

    Applicant must possess: strong clinical background in Wound Care and HBO; thorough knowledge of wound care case management and discharge planning; current knowledge of current standards of practice in Wound Care; proficiency in MS Office Suite; excellent communication, interpersonal, organizational and documentation skills; positive attitude; ability to work well with others; strong team building and mentoring skills; ability to demonstrate commitment to excellence in Wound Care; ability to provide direct treatment of wounds (i.e. application of Unna boots, multiple layers of bandages, ointments and wound dressings; continual monitoring of wounds for signs of infection; providing treatment protocols to ward off skin irritation and infection, etc.).
