  • Overview

    Assists home care patients in reaching maximum performance while adjusting to disabilities. Treats disabled patients using standard therapy procedures. Evaluates patients’ capabilities, revises therapy programs as patients’ progress is noted and instructs patients and families in therapeutic procedures.


      • On the initial visit and the revisits thereafter evaluates patients’ physical and functional status in order to develop an appropriate care plan.
      • Initially and ongoing, observes and interviews patients, reviews medical histories including physicians’ diagnoses and prescriptions.
      • Establishes short and long term goals in conjunction with the patient and formulates treatment plans consistent with the goals.
      • Discusses the treatment plan with the M.D.
      • Determines the appropriate treatment modalities and equipment necessary for successful completion of the treatment plan.
      • Utilizes the agencies’ clinical guidelines in the development of the treatment plan.
    • COORDINATES PATIENT SERVICES                                                                     
      • Performs active chart review to ensure coordination with other disciplines.
      • Documents telephone calls to physician when there is a change in patient condition.
      • Documents case conferences with other disciplines every two weeks.
      • Assumes ongoing therapy and coordination responsibilities for an assigned caseload of patients.
      • On an ongoing basis reviews all documentation placed into the computerized record. This includes but is not limited to nursing notes and reports from other types of therapists.
      • Understands and utilizes information regarding the home care payment system to ensure correct reimbursement from payors on every medical record.
      • Forwards all pertinent patient information as it is received to the managed care coordinator to facilitate reimbursement.
      • Reports off to the supervisor or designee the status of the patients under their care and any intervention needed prior to vacation or extended days off.
      • Completes all necessary paperwork within 24 hours of the start of care.
      • Communicates the notebook as per policy to ensure the exchange of patient information.
      • As per agency need, accepts new cases.
      • Schedules all revisits within the plan of care.
      • Contacts the patient with a visit date by the end of the work day.
      • Informs the office of the patients scheduled initial visit date at the time of acceptance of the case.
      • Reliable in following through on assignments and appointments with patients for treatment and is conscientious.
      • Together with the patient develops a discharge plan.
      • Secures coverage for patients while on vacation to ensure progress toward goals.
      • Maintains current knowledge regarding trends in the profession.
      • Accepts assignments based on demonstrated competency to provide care to age and gender specific population.
      • Participates on and is aware of the agency’s’ performance improvement plans and committees.
      • Attends the annual therapists meeting.
      • Complies with Department of Health regulations regarding care to patients’ and documentation of it as to not put the agency into a deficiency mode.


        Knowledge of anatomy and physiology and physical therapy treatments and modalities.

        Current New York State license as a Physical Therapist.

        Approximately 12 months on the job experience.

        Analytical skills.

        Interpersonal skills.

        Ability to walk and stand for up to 50% of work time, and lift and turn patients.

        Ability to concentrate and pay close attention to detail for up to 75% of work time.
