  • Overview

    The Wound Care Nurse Practitioner Coordinator is responsible for providing specialty clinical care for the Wound and Hyperbaric Center. The Wound Care Nurse Practitioner Coordinator’s role includes the application of evidence-based assessments, diagnosing, prescribing, treatments, and education for patients in collaboration with the Wound Care Center team. He/she consults and assists the Wound Center’s clinical staff with monthly chart reviews, on-going process improvement plans, infection control protocols and all components of patient care coordination. The Wound Care Nurse Practitioner Coordinator functions as a liaison for the Inpatient Wound Care services within the BMHMC organization and serves on hospital committees as requested. He/she is additionally expected to attend monthly department meetings as part of his/her involvement in the growth and operational success of the Wound Care center program. With a patient centered approach, he/she will promote health prevention initiatives and provide healthy lifestyle coaching for the patient population.



    ·        Assesses patient status by obtaining health history through patient/family interview. Assesses co-morbidities, risk factors, family history, and psychosocial aspects of the patient’s presentation.  


    ·        Performs comprehensive evaluation of wound staging according to the Wagner Scale and reports such data in proprietary EMR, paper chart, as well as, on hospital’s dictation platform. Completes all clinical documentation in compliance with hospital mandated timeline.


    ·        Prescribes a plan of corrective care including diagnostic tests, medications, minor surgery (such as debridement), and diagnostic and corrective procedures. 


    ·        Implements and completes specific treatment plans according to healing protocols outlined in BMHMC and Healogics’ clinical guidelines, policies and procedures.

    ·        Interprets laboratory tests and diagnostic studies according to established hospital protocols and

    consults with physicians as necessary.

    ·        Administers medications in compliance with state law and clinic guidelines.  Such medication may include oxygen, oral antibiotics, local anesthetic gels, bioengineered skin substitutes.


    ·        Provides patient and caregiver education pertinent to the promotion of wellness.


    ·        Seeks other information as needed including collaboration and consultation with physicians and other clinicians for evaluation of illnesses and managing acute and long-term medical needs of patients. 


    ·        Complies with all local, NY State and federal laws, regulatory agency standards and organizational policies and procedures (e.g. CMS, DNV, NCQA, HIPAA, DOH, OSHA, etc.)    

    • Provides and participates in educational in-services for nursing  staff.                                                       
    • Provides pre-cepting and supervision to residents, students    and other staff as requested.


    • Provides monitoring and continuity of care between physician visits, triaging patient telephone calls and

              providing consultation.                                                             


    • Participates in Performance Improvement (PI) activities including tracers, chart reviews,

               medical surveillance reporting and   actions related to continuous      

                audit  preparedness.                                                                                      


    • Serves on hospital committees when requested and supports the in-patient care continuum for BMHMC.                                                                                                           


    • Agrees to attempt to provide vacation or absentee coverage for Physicians to ensure continuity

              of care within clinic.









    • Professional Knowledge of nursing theory and practice acquired through completion of a
    • Bachelor of Science in Nursing and Adult Nurse Practitioner program
    •   Current New York State License as a Nurse Practitioner
    • Approximately two to three years on the job experience, including demonstration of progressive responsibility
    • in unit-based nursing management and staff education
    • Complete Wound Care and Hyperbaric Oxygen Therapy training
    • Analytical ability to assess and develop solutions to complex clinical situations   and/or area problems
    • BLS certification
    • Ability to communicate effectively in written and verbal format
    • Current DEA License
    • Completion of 40 hour primary course in Hyperbaric Oxygen Medicine
