Responsible for providing primary care services, including assessing, diagnosing, prescribing, treating, and educating patients in a NCQA Patient Centered medical home (PCMH patient centered approach to patient care. Promotes health prevention initiatives and healthy lifestyle for patient population. Educates patients, families and the community to promote wellness and prevention of diseases. Leads team to provide quality care in primary care setting. Provides clinical training to residents, Nurse practitioners, medical students, and others.
Schedule of 8am-4pm 2 days a week and
Saturdays. 9AM -1PM
Current New York State License as a Physician
